最近経験した結石を伴う原発性上皮小体機能亢進症の5例

RECENTLY EXPERIENCED FIVE CASES OF PRIMARY HYPERPARATHYROIDISM, CAUSING RENAL STONE

はじめに

　原発性上皮小体（副甲状腺）機能亢進症は従来比較的稀な疾患とされており，大阪大学で自験例52例をまとめて発表^1）して以来は多数例を一機関で（特に泌尿器科領域で）報告していることが少ない。本疾患の疑いをもつていろいろ調べてみてもなかなか自信をもって診断することができないのは，本疾患に対する経験が少ないからであろうが，一旦自信をもつて診断し手術に成功すれば意外に多くのかくれた本症を見出しうることがわかつたので，その経験を述べるとともに，手術時の上皮小体の検索，組織所見，とくに低倍率の組織写真を供覧し，腺腫，過形成，癌腫などの区別についての所見を記載する。

Five cases of primary hyperparathyroidism causing renal stone, which have been experienced by us these one and half years, are presented.

As the parathyroid changes are minimal in primary hyperparathyroidism with renal stone, the hor-monal studies are not conclusive. Therefore, we propose the dual determination of serum calcium and phosphorus as the only reliable data to suggest the primary hyperparathyroidism. The rationale of the exploration of the neck is discussed.
