腹直筋鞘を利用した尿管瘻術

URETEROSTOMY USING THE SHEATH OF THE RECTUS MUSCLE

緒言

　尿管皮膚瘻術は，膀胱癌における尿路変更術としては全身状態の良くない症例に行なわれているが，従来より諸家によりカテーテルを必要としない尿管皮膚瘻術が工夫されている。

　尿管皮膚瘻術における尿管狭窄の原因として，尿管栄養血管の損傷，筋肉，皮下脂肪，皮膚の線維化による圧迫があげられているが，われわれは後者を重視し，直接尿管断端周囲に筋肉，皮下脂肪，皮膚が接しないようにすることにより，狭窄が防止されるのではないかという見地より，腹直筋鞘を利用する方法を，肝硬変合併症例，および既往に脳卒中のある症例に行ない，カテーテルを必要とせずに管理可能となつているので，ここに報告する。

The stomal stenosis is one of the main complication of the cutaneous ureterostomy. The injury of the ureteral blood supply and the compression of the muscle, subcutaneous fat and skin have so far been regarded as the major causes of the stomal stenosis. We have regarded the latter as primary cause, and the ureterostomy using the sheath of the rectus muscle was performed in 2 patients with bladder cancer.

The location of the stoma in this method is anatomically limitted to the portion between the semicircular line and umbilicus upon the abdominal midline.
