腹直筋鞘血腫の1例

A Case of Rectus Sheath Hematoma

　腹直筋鞘血腫の1例を経験した．症例は78歳の男性で，咳嗽を契機として右下腹部痛を発症した．右腋窩-大腿動脈バイパス術の既往があり，来院時著明な高血圧を認めた．エコー検査でhypoechoic areaが，CT検査でhigh density areaが腹壁直下に認められた．腹直筋鞘血腫の診断にて保存的に加療した．本症は下腹壁動脈の破裂によって生ずる稀な疾患で，以前は急性腹症として手術適応となることが多かったが，近年は画像診断の進歩により確定診断がなされることが多い．原則的には保存的治療の可能な疾患である．

The following is a rare case of rectus sheath hematoma. The patient was a 78-year-old man with a past history of axillo-femoral bypass operation. He contracted a cold, and when he coughed severely, acute lower abdominal pain occured. Abdominal echography and CT scanning confirmed the presence of a hematoma in the rectus abdominis muscle. Before, patient was diagnosed as acute abdomen and surgery was perfor-med. Recently, accurate diagnosis has been made easily by echography or CT.
