残存尿管から発生した膀胱腟瘻の1例

A SPONTANEOUS VESICO-ECTOPIC URETERO-VAGINAL FISTULA IN A GIRL

緒言

　幼小児の膀胱腟瘻は，外傷性あるいは医原性（主として膀胱頸部切除術に基因^1,2））のものを除くと極く稀であり，文献的にcongenital vesico-vaginal fistulaとして今まで4例の報告があるのみである^3〜6）（第1表）。手術操作，術後感染などに直接原因を求めることのできないspontaneous　vcsico-vaginal fistulaの報告例は今までには見られていないようである。

　今回われわれは，膣内異所開口尿管による尿管性尿失禁を主訴とした女児に対し，腎摘出術を行ない，術後4年間は尿失禁は消失していたが，4年後に再度尿失禁が出現するようになり，諸検査の結果，尿失禁の原因が膀胱—残存尿管—膣瘻によるものであることがわかり，瘻孔閉鎖術にて尿失禁を消失せしめ得た8歳女児例を経験したので報告する。

A case of spontaneous vesico-vaginal fistula through an ectopic ureteral stump in 8 years old girl was presented.

She had a history of urinary incontinence between a normal voiding, pathognomonic of an ectopic ureter at 3 years old and had a simple nephrectomy for the right single ectopic ureter opening into the Gartner's duct at our clinic. She had been free of incontinence and had remained dry until the age 7 when she started to note a similar type of incontinence. Excretory urogram, cystoscopy and cys-tometrogram were not revealing, therefore further examination were not performed.
