針麻酔下での経尿道的手術

TRANSURETHRAL OPERATION UNDER ACUPUNCTURE ANESTHESIA

緒言

　針麻酔下での手術成功の報が中国より伝えられて以来^1），日本でも針麻酔下での手術例が報告され^2〜5），術中患者の意識が明瞭であることおよび術後経過の良好なことが注目されている。

　針麻酔の鎮痛機序および針麻酔時の生体の変化も研究されているが^6,7），十分な鎮痛を得るためにはツボと言われる部位に刺激を与え続け，独特な「だるい，しびれる，重い，はれぼつたい」と言つた「得気」が持続しなければならないと考えられている^8）。針麻酔は実体が解剖学的に不明なツボおよびその刺激によつて生じる「得気」という患者の感覚に基礎を置いているので，術者も患者も必ず鎮痛が得られるという確信のもてないことおよび十分な筋弛緩作用が得られないことが薬剤麻酔技術の発達した現在では特別の場合を除いては針麻酔を選択することを躊躇させる原因であると思われる。われわれは薬剤麻酔がためらわれる患者に筋弛緩を必要としない手術を行なう場合には針麻酔は十分に応用できると考え，針麻酔下で8例の経尿道的手術を行なつた。

Eight cases of transurethral operation under acupuncture anesthesia were experienced. Transurethral ureteromeatotomy was performed in 2 patients without pain. In 5 cases of transurethral electroresection or electrocoagulation of bladder tumor, two had no pain but three, with prostatitis. complained pain in the urethra, when cystoscopy was advanced to the enlarged prostate. In 1 case of transurethral electroresection and electrocoagulation of urethral tumor, analgesic effect was successful in the proximal portion of urethra, but not sufficient in the distal portion.
