A case of hydronephrosis due to aberrant blood vessel of the kidney in a 22 year old male was reported. The aberrant blood vessel was a small artery branching from the midportion of left renal artery distributing the lower pole of the left kidney. This aberrant blood vessel was oppressing the ureteropelvic junction from the anterior side and therefore the blood vessel was ligated and dissected together with left nephropexsy which was performed since it had almost no relation to the renal blood flow. Hydronephro sisshowed remarkable improvement after surgery.
