血漿PSPの臨床的検討

THE CLINICAL STUDY OF THE PLASMA PSP CONCENTRATION AS A TEST OF RENAL FUNCTION

緒言

　泌尿器科領域において，総腎機能を定量的に評価することは臨床上不可欠のことである。現在広く一般に用いられているクリアランス試験，薬物排泄試験などは採尿という操作が必須である。尿路死腔を有する場合（特に上部尿路死腔），または尿瘻や尿路変向のために正確な採尿が不可能な場合，または高度の血尿の場合には正確な値を知ることができないという欠点がある。また，レノグラム，排泄性腎盂撮影でも腎機能を推量することはできるが，数量的に表現することはまだ一般化されていないのが現状である。われわれはGault^1〜5）によつて発表された血漿PSP減衰率（PSP-lndex,PSPIと略す）を測定し，検討を行なつたのでその成績について報告する。

The PSP index (PSPI) and the plasma PSP concentration were measured in 42 cases.

The sixty-minute plasma PSP concentration (PSP₆₀) showed the highest correlation with PSPI, creatinine clearance and PSP excretion test in the patients with no dead space in the upper urinary tract. However, the PSP₆₀ showed no correlation with PSP excretion test in the patients with dead space in the upper urinary tract. The zero time value by backward extrapolation were calculated at 0. 47 ± 0.01 mg/dl, and so-called "PSP space" is comparable to 21.3% of the body weight.
