選択的副腎静脈撮影により確定診断しえた原発性アルドステロン症の1例

A CASE OF PRIMARY ALDOSTERONISM CONFIRMED BY THE SELECTIVE ADRENAL PHLEBOGRAPHY

緒言

　1955年Connにより原発性アルドステロン症が見出され，本邦でも1957年に鳥飼らにより第1例が報告されてから今日まで143例以上の原発性アルドステロン症が報告されているが，未だに疾患の定義そのものや診断上の問題点が多く解決されないままで残つている。

　近年，副腎静脈撮影が副腎腫瘍ことに原発性アルドステロン症診断に有用であるとの報告に接するようになつたが，われわれも最近原発性アルドステロン症の1例に副腎静脈撮影をこころみ，術前確定診断を下すことにより手術を容易に行ない得た1例を経験したので報告する。

A case of primary aldosteronism in a 46 year old female complaining loss of appetite, weakness of both lower extremities, thirst, polydipsia and polyuria, was reported. Objective findings revealed low urinary sodium and potasium ratio, positive aldacton reaction, drop of renin activity by limiting salt intake and increased urinary aldosteron. An acron-sized mass was outlined at the upper pole of right kidney by X-ray examination including I.V.P. and tomogram combined with P.R.P. but since the diagnosis of adrenal tumor could not be established right adrenal venography was done.
