腎洞部腫瘍の手術について—解剖学的関係

SURGERY ON RENAL SINUS MASS : ANATOMICAL RELATIONSHIP TO VESSELS

緒言

　ときにみられるaberrent vesselを別として，腎門部に出入りする動静脈はそれぞれ1本である。この1本の動静脈が腎洞部で次々に分岐していくのであるが，この点については上腎杯頸部圧迫症候群，pyelocalicotomy，あるいは腎切石，腎部分切除術と関連して多少関心がもたれるようになつてきたが，一般的にはあまり注目されていない。筆者は最近の手術経験から，腎洞部のmass lesionにおけるこの解剖学的関係の重要性について述べる。

The renal artery, outside of the kidney is single, divides into 5 segmental arteries within the sinus which are the posterior, apical, upper,middle and lower. It is the same with the vein. The anatomical relationship is important in cases of renal sinus mass lesion.

I have recently operated on 2 patients both of whom had large mass in right renal sinus. The mass distorted the vessels besides the collecting system. On abdominal transperitoneal approach one of the branches was at first mistaken for main renal artery.
