乳児嚢胞腎の1例

POLYCYSTIC KIDNEYS IN AN INFANT: CASE REPORT

Ⅰ．はじめに

　腎に発生する嚢胞性疾患は，その多くが先天性の発育異常とみなされており，病因について，多様の見解が述べられているとともに，多くの分類が試みられている。中でも，嚢胞腎は，両腎性の嚢胞性疾患で，発生頻度が比較的高く，家系的な素因が重視され，進行性の腎機能障害を示すことなどから，古くから興味を持たれてきた。そして，死産児または新生児の剖検によつて発見される嚢胞腎（幼児型）と，成長してから発見される嚢胞腎（成人型）とでは，種々の点で差異があるといわれ，また，小児の症例もまれに報告されている。われわれは，生後8ヵ月の乳児嚢胞腎の症例を経験したので報告し，幼児型と成人型との差異について考えてみたい。

Case is a 8 months old male with nothing particular in the family and past history. Bilateral flank mass was pointed out during health check up at the Health Center which brought him to the Urolo-gical clinic of Niigata University Hospital. IVP revealed poor excretion of dye from both kidney with pyelogram and PRP showed both kidneys enlarged to small fist size. Left kidney was exposed through flank incision and the kidney surface was irregular, resembling fetal lobulation but no cyst was found although the consistency was spongelike by palpation. Surgery was terminated after biopsy of the kidney.
