重篤な陰茎絞扼症に対する陰茎形成術

RECONSTRUCTIVE PLASTIC SURGERY FOR SEVERE STRANGULATION OF THE PENIS

Ⅰ．はじめに

　産業災害あるいは交通事故の増加とともに以前にはみられなかつた種々の傷害が発生している。泌尿器科領域においても腎損傷，更に下腹部あるいは骨盤部の外傷等により膀胱破裂，尿道損傷，あるいは陰茎外傷等もしばしばみられるようになつた。これらとは別に従来，何かの目的で故意に行なわれる陰茎外傷もあとをたたない状態である。その一つである陰茎絞扼症は本邦にても比較的稀な症患ではあるが，最近，我々は重篤な本症，即ち陰茎表皮および尿道の完全断裂更に陰茎海綿体まで損傷された症例に対し陰茎形成術を施行，陰茎の再建に成功したので報告する。

Plastic surgery of the penis was performed in a 29 years old boy whose penis was just connected by a bridge like scar of 1.5cm in diameter due to strangulation of the penis by a metal ring 13 years ago which was left alone for 2 years until the ring came off by disintegration with trauma of the penile skin and corpora cavernosum and complete rupture of the urethra forming scar of a wide area. This successful case with healed urethral fistula, almost normal appearance of the penis and complete errection after surgery, was reported.
