MÜLLERIAN DUCT CYST: Report of a Case

　ミューラー管は泌尿生殖器原基の一部で，本来ならば女子では発展分化して卵管，子宮および腔上部を形成するが，男子では退化し睾丸附属体および男性子宮としてわずかに遺残するのみとなる筈である。男子において胎生期ミューラー管の退化過程が障害されて，その末梢部が嚢胞様に拡張されて残ることがあり，文献的にはEnglischが1873年に1剖検例でこの状態を発見し，Cystic Mul-lerian duct remnantと命名したのが最初の報告のようである。以後同様の報告が散見されているが極めて稀れなものと思われ，本邦では2例の報告をみるのみである。我々は最近，無症候性血尿を主訴とした膀胱癌患者において，直腸指診でいわば偶然に膀胱底部の嚢胞を発見し諸検査の結果ミューラー管嚢胞と判明した1症例を経験したので報告する。

A 54 years old man with a history of haematuria for 18 months, visited our clinic.

Cystoscopic examination revealed elevation of the trigone and a papillary tumor at lateral wall of the bladder. Rectal palpation revealed normal prostate and prune sized cystic mass just above the prostate. Transrectally, 20ml of yellow-brown fluid was aspirated from the cyst, and contained no spermatozoa nor parasite. X-ray examination showed the cyst located between the laterally dislocated seminal vesicles and posterior urethra. The patient's sex chromatin was negative, and sex chromosom was XY.
