摘出に難渋した磁性体を核とした膀胱異物結石

A case of magnetic foreign body with formation of urinary bladder stone,involving great difficulties in removal

症例は62歳男性。主に下腹部痛を主訴に泌尿器科を受診。当初尿道炎として抗菌剤投与するも軽快せず，6日後再診となる。再診時のエコー，KUBでゴルフボール大の膀胱結石と結石内部の数珠状異物の所見を認めた。後日内視鏡下膀胱砕石術および異物除去術を施行したが，異物は小磁性体物質よりなっており摘出に難渋した。外来診療で難治性の尿路感染症を診た場合，異物や結石の存在を念頭に置く重要性が再認識された。

A 62-year-old male with pyuria and tenesmus visited our hospital and was treated as simple urethritis at first. but later proved to be vesical stone around foreign body by KUB and echog ram. Then we performed endoscopic lithotripsy and observed the foreign bodies made of magnetic materials as core of stone. We could not remove them at one time,but successfully removed each piece one by one transurethrally using optical forceps. Foreign body in the urinary bladder should be taken into consideration in case of prolonged urinary tract infection.
