症例は74歳男性。発熱，左側腹部痛を主訴に当科を受診した。腹部CT検査にて左尿管腫瘍と診断。左腎尿管全摘除術，膀胱部分切除術および腎門部リンパ節郭清術を施行した。病理結果は，腫瘤を伴うIgG4陽性形質細胞の浸潤による後腹膜線維症と診断した。他臓器にIgG4関連自己免疫性疾患を示唆する所見は得られていない。診断時，悪性腫瘍を疑われることが多いが，血清IgG4を計測するなど，本症例を念頭におくことが肝要である。

A 74-year-old man was referred to our hospital for a high fever and left flank pain. CT scan and MRI revealed the left hydronephrosis and left ureteral tumor. After the treatment for the left pyelonephritis by the drip infusion antibiotics,we performed left nephrou-reterectomy,partial cystectomy and lymphadenectomy. Histopathological diagnosis was IgG4-related retroperitoneal fibrosis. It is necessary to acknowledge the relationship between retroperitoneal fibrosis and of IgG4-related systemic disease and the measurement of serum IgG4 and tissue immunostaining for IgG4 should be considered in the diagnosis of retroperitoneal fibrosis.
