腫瘤触知を主訴とした後腹膜線維症

A case of retroperitoneal fibrosis with lower abdominal mass

　症例は62歳，女性。下腹部腫瘤，左水腎症の精査目的にて当院を受診した。画像所見では左腎周囲腔から左後腹膜腔に連続して広がる腫瘤が認められた。悪性リンパ腫を考慮し，経皮的針生検を施行した。病理組織学的検査結果は線維組織との診断であった。しかし，悪性リンパ腫が否定できないため開放生検も施行したが，検査結果は針生検と同じであり，われわれは後腹膜線維症と診断した。ステロイド治療を開始し，自覚症状，画像所見とも著明に改善を認めた。

　A 62-year-old woman was referred to our hospital for a low abdominal mass and left hydronephrosis. CT scan and MRI revealed a tumor extending in left retroperitoneal space. Needle biopsy was performed, because of malignant lymphoma was suspected. Pathological diagnosis was fibrous tissue. However malignant lymphoma could not be denied. Open biopsy revealed same diagnosis. We finally made the diagnosis of retroperitoneal fibrosis. Symptoms improved immediately after the start of steroid therapy. The tumor was significantly reduced as well.
