前立腺神経内分泌癌の1例

A case of neuroendocrine carcinoma of the prostate

　84歳，男性。前立腺癌の診断で他院にてホルモン療法を施行されていた。ホルモン治療導入後，1年2か月経過した時点で肉眼的血尿を認めるようになり，当院を受診した。受診時，下腹部膨満と両下肢の強度の浮腫を認め，腹部骨盤腔CTで，骨盤内に巨大な腫瘍を認めた。前立腺針生検を施行したところ神経内分泌癌と診断された。

　Neuroendocrine（NE）differentiation associated with adenocarcinoma of the prostate has received increasing attention in recent years due to its prognostic and therapeutic implications. Recent evidence indicates its possible role in tumor proliferation,progression and development of androgen refractory state. However,pure NE carcinoma of the prostate is extremely rare,and is characterized by tumor progression,androgen-independent state and poor prognosis for which there is currently no effective therapy. We report a 84-year-old male with lower urinary obstructive symptom in whom NE carcinoma of the prostate was histologically confirmed following antiandrogen therapy.
