膀胱タンポナーデの臨床的検討

Clinical analysis on 39 patients with bladder tamponade

1999～2004年までの6年間に膀胱タンポナーデにて当科で治療した39例に対し，年齢，性別，原因疾患，治療について検討した。男女比は4：1であり，70歳代が最も多くみられた。原因疾患としては膀胱癌（7例），前立腺肥大症（7例），TUR後出血（6例）が多くみられた。治療としては，まず尿道カテーテル留置と凝血塊除去を行ったが，軽快しない15例（膀胱内腫瘍9例，前立腺肥大症4例，TUR後出血2例）に対し経尿道的手術を行わなければならなかった。膀胱タンポナーデに対する治療では，保存的治療でコントロールがつかない場合，外科的治療への変更を素早く行うべきであると思われた。

We evaluated 39 patients with bladder tamponade in our department for six years from 1999 to 2004 about age,sex,primary disease and treatment. Men and women ratio was 4：1 and most frequently occurred in 70 years age group. Primary disease was bladder neoplasm（7 cases）,benign prostatic hyperplasia（7 cases）and hemorrhage after transurethral operation（6 cases）. For primary treatment we performed urethral catheterization with clot removal in all cases,but we ought to perform transurethral operation for 15 cases（tumor in urinary bladder：9 cases,benigh prostatic hyperplasia：4 cases,hemorrhage after transurethral operation：2 cases）. We feel quick decision to transurethral operation is mandatory.
