ヂューリング疱疹状皮膚炎を思わせた落葉状天疱瘡

PEMPHIGUS FOLIACEUS SIMULATING DERMATITIS HERPETIFORMIS DUHRING

はじめに

　ヂューリング疱疹状皮膚炎と天疱瘡はきわめて近似の疾患で，時にいづれか診断に迷う場合がある。私は初め典型的な疱疹状皮膚炎と考えられたが，組織的には表皮内水疱であり，後に落葉状となつて，重篤な脳症状を伴つた1例を経験したので報告する。

A 24-year-old unmarried woman of this disease with the clinical picture of dermatitis herpeti-formis Duhring was reported. She had annularly arranged bullae with itching, positive KJ test, and negative Nikolski's test. Blood eosinophils were 5-12% and those in the blister were 39%. Histologic specimen showed the intraepidermal bulla. The disease changed the clinical picture to pemphigus foliaceus later accompanied by severe brain symptoms.

This case proved the opinion that the early stage of pemphigus foliaceus takes the clinical picture of dermatitis herpetiformis. The author agrees with the opinion that the diagnosis should not be given clinically, but by histologic findings.

The brain symptoms might be due to dehydration and disturbed mineral metabolism of the body fluid.
