The DMSO-containing KOH preparation, reported by Zaias and Taplin (1966), was compared, clinically and experimentally, with classical plain KOH preparation. The recommended reagent for clinical use contained 20% KOH by weight in 30% DMSO solution. Material from the lesion was treated with DMSD reagent by the same technique as KOH preparation, but heating was omitted. The advantages of the DMSO preparation was as follows : 1) Horny material was softened easily without heating. 2) The contrast between fungual structures and horny cells and the transparency of horny cells was more marked in DMSO preparation than in KOH preparation. 3) The time necessary for fungus detection was shortened, namely, fungus could be detected within one minute in scale and within several minutes in nail. 4) It made thick material so transparent that the preparation without pressure made possible to observe parasitic form of fungus in natural condition. 5) DMSO preparation was harder to dry than KOH preparation. It could be used for microscopic study even when it was left in air for 24 hours.
