猫ひっかき病の2症例

Cat Scratch Disease:Report of Two Cases

　はじめに

　頭頸部領域のリンパ節腫脹をきたす疾患は種々あるが，急性感染症状に乏しく，また持続するリンパ節腫脹は悪性腫瘍との鑑別に苦慮することが多く，確定診断のために生検を余儀なくされることがある。猫ひっかき病は，猫との接触の後に局所リンパ節腫脹をきたす疾患で，リンパ節腫脹は腋下，頭頸部領域に多いとされる^1,2）。猫ひっかき病は現在確立した診断法がなく，原因不明の頭頸部リンパ節腫脹をみた場合に同疾患を鑑別疾患の1つとして考慮する必要がある^3〜5）。今回われわれは，リンパ節生検で病理組織中に桿菌が証明された猫ひっかき病2例を経験したので，若干の文献的考察を加え報告する。

We report two cases of cat scratch disease with lymph node swelling of head and neck region. A 13-year-old female presented with a submental swell-ing and a 23-year-old female presented with aswelling of the left parotid gland. Although both of them had a history of contact with cat and scratch wound, malignant tumors had to be considered as a differential diagnosis. Pathological specimens revealed granulomas and scattered becilli, stained with Warthin-Starry staining. An enzyme im-munoassay for the detection of IgG and IgM anti-bodies to Bartonella henselae was negative in these cases.
