喉頭に原発した腺様嚢胞癌の1例

Adenoid Cystic Carcinoma of the Larynx:Report of a Case

　はじめに

　腺様嚢胞癌は唾液腺に好発するが，喉頭における発生は比較的稀である。最近われわれは，最終的な診断確定までに長期を要した喉頭原発の腺様嚢胞癌の1例を経験したので，その発生頻度，診断および治療法，予後などについて，文献的な考察を加えて報告する。

A 62-year-old man complained of hoarseness of two months duration. On January 1996, our first fiberscopic findings were unremarkable. On Septem-ber, left vocal cord fixation and on December bila-teral vocal cord fixation was recognized respect-ively. On January 1997, direct laryngoscopy and biopsy were performed. The biopsy was positive for small cell carcinoma of the larynx (T4NOM0). He underwent total laryngectomy and thyroidectomy and postoperative irradiation (70Gy). Pathological study of the surgical specimen revealed adenoid cystic carcinoma of the larynx (transglottic type, grade III). He has remained well for the last 13 months without any evidence of recurrence or metastasis.
