猫ひっかき病の1症例

A Case of Cat Scratch Disease

　はじめに

　猫ひっかき病は1950年にDcbre^1）によって最初に報告され，1953年には浜口ら^2）によってわが国での第1例の報告がされている。この後，特に欧米では患者数が多いこともあり数多くの研究や報告がある^3,4）。しかし本邦では報告が欧米に比較して少ない^5）うえ，多くは外科^2）や小児科^6）の報告で，耳鼻咽喉科からの報告は少ない^7）。

　耳鼻咽喉科の一般診療において頸部腫瘤を主訴とする患者は少なくない。そのうち頸部リンパ節腫脹はかなりの割合を占める^8,9）が，猫ひっかき病によるリンパ節炎は比較的稀である^10）。

　今回われわれは頸部腫瘤を主訴として来院した猫ひっかき病の1例を経験したので報告する。

A 44-year-old female complained of a neck swelling and pain. On admission she had abra-sions on both preauricular regions and lymph-adenopathy of the right cervical portion. She had kept cats for several years. Histologically the excised tissues were characterized by the granulo-matous changes with epidermoid cells and giant cells. Pleomorphic microbacilli were often observed in the giant cells by Warthin-Starry staining. The present case showing the regional lymphade-nopathy, intimate exposure to a cat and inocu-lation site was identical to “The Rule of Five” in cat scratch disease proposed by Carithers.
