Brachytherapy with Iridium-192 Thin Wire for Head and Neck Carcinoma

　はじめに

　頭頸部癌の治療には，腫瘍の局所制御と同時に機能・形態の温存が重視され，放射線治療は大きな役割を占めている。中でも²²⁶Raによる舌癌の小線源放射線治療（brachytherapy）は，根治的治療法として確立された方法である^1,2）。

　近年，密封小線源は²²⁶Raから¹³⁷Csや¹⁹²Irへと切り替わりつつあるが^3,4），¹⁹²Ir thin wireは²²⁶Raや¹³⁷Csの針や管の形状による線源では困難な部位への刺入も可能であり，本邦においてもその利用が報告されてきている^5,6）。

　今回われわれは，¹⁹²Ir thin wireによる頭頸部癌の小線源放射線治療を行い，良好な治療効果が得られたので報告する。

Six patients with head and neck carcinoma were treated by interstitial implant of Iridium-192 thinwire using plastic tube (angio-catheter) technique.

Complete response (CR) was obtained in five cases and partial response in one case. Local recur-rence was occurred in one of CR case. Late injury was observed in two cases.

Iridium-192 thin wire interstitial implant appear-ed to have a potential to obtain a better local control for head and neck carcinoma, especially for those in faucial pillar or tonsillar region.
