ツベルクリン反応が陰性であった頸部リンパ節結核の1症例

A Case of Tuberculous Cervical Lymphadenitis with Negative Tuberculin Reaction

　はじめに

　結核は，本邦において新規発生患者数が1997年に38年ぶりに前年を上回り，厚生省より非常事態宣言が発令されるなど，再興感染症として注目を集めている。耳鼻咽喉科領域において，頸部リンパ節結核は頸部腫瘤の鑑別疾患の1つとして常に念頭に置く必要のある疾患である。一般に頸部リンパ節結核が疑われた場合には，ッベルクリン反応が簡便で有用な検査である。

　今回われわれは，ツベルクリン反応が陰性で，最終的には摘出したリンパ節の病理組織学的所見より診断が確定した頸部リンパ節結核の1症例を経験した。本症例の経過およびツベルクリン反応が陰性であった原因も含め，文献的考察を加えて報告する。

In a patient with tuberculous cervical lymphadenitisbut no pulmonary tuberculosis, tuberculin reaction was negative. Definitive diagnosis was made by the patho-logical examination of open biopsy. In the case of tuber-culosis, tuberculin reaction is easy and important exami-nation to make diagnosis. But tuberculin reaction is influ-enced by various patient's factcr, therefore we can't deny tubeculosis even if tuberculin reaction is negative.
