A 23-year-old male with transverse fracture of tem-poral bone presented with severe facial palsy on the frac-tured side, deafness and canal paresis. Total decompres-sion of the facial nerve via enlarged translabyrinthine approach was performed. Two years later, facial palsy improved moderately. For a traumatically deafened case with facial palsy, the enlarged translabyrinthine approach is recommended as rather good approach to decompress the total facial nerve, because of its wide view and space for surgical maneuver around the internal auditory canal and the labyrinthine portion.
