鼻中隔に発生した筋上皮腫の1例

A Myoepithelioma of the Nasal Septum:Report of a Case

　はじめに

　筋上皮腫は外分泌腺の介在部に存在し筋上皮細胞に由来する稀な腫瘍である^1）。そのため，本症の多くは大唾液腺に発生するとされているが，全唾液腺腫瘍に占める割合は約1％に過ぎない^2）。さらに，本腫瘍が鼻・副鼻腔領域に原発することは極めて稀であり，これまでに国内外を含めて9例の報告をみるのみである^3〜11）。

　今回われわれは，鼻中隔より発生した筋上皮腫の1例を経験したので，若干の文献的考察を加えて報告する。

A rare case of myoepithelioma originated from the nasal septum in a 31-year-old female who com-plained of recurrent epistaxis from the left nasalcavity is reported. CT and MRI examinations revealed a solid mass lesion occupying the anterior part of the left nasal cavity. Radical extirpation of the mass was performed by extranasal approach. Histopathological examination showed that tumor cells had spindle-shaped cytoplasm and round nu-clei. Immunohistologically, the tumor was positive for S-100 protein, vimentin, α-SMA, and cytokera-tin. From these findings, myoepithlioma was diagnosed. She has no evidence of recurrence at 14 months'follow-up.
