顔面神経を切除した耳下腺癌における皮神経付き遊離前腕皮弁の使用経験

A Case of Parotid Gland Carcinoma:The Defect Repaired with a Forearm Flap with the Lateral Cutaneous Nerve

　はじめに

　耳下腺癌の治療は進行度，組織型に応じた腫瘍の切除が重要である。同時に欠損部位の再建も重要である。特に耳下腺癌では，顔面神経を合併切除しなければならないことが多く，可能な限り神経再建を図る必要がある。

　今回われわれは，顔面神経再建に外側前腕皮神経付き遊離前腕皮弁を用いて，進行耳下腺癌の顔面神経および欠損整復を行った1例を経験したので，若干の文献的考察を加えて報告する。

In a case of parotid gland carcinoma, a wideresection of the right parotid gland, and the right radical neck dissection were performed. To repair the defect of skin and the facial nerve, we used the forearm flap with the lateral cutaneous nerve of forearm. Postoperative radiotherapy was performed (70 Gy). There was no evidence of recurrence or metastasis at the time of hospital-discharge. However a long term follow up is necessary, because it is reported that prognosis of such case is very poor.
