退院直後に急性腎不全を発症した鼻・副鼻腔手術の1症例

A Case of Acute Renal Failure Following Sinus Surgery

　はじめに

　基礎疾患がなく全身状態良好な患者の場合，比較的侵襲が低い鼻・副鼻腔手術の術後に重篤な合併症が伴うことは少ない^1）。

　今回われわれは，鼻・副鼻腔手術後に急性腎不全を発症し，早期に診断と治療を行うことで救命し得た症例を経験したので報告する。

A 50-year-old man had a postoperative maxillary cyst and underwent Caldwell-Luc sinus surgery. Thepatient developed various symptoms similar to Toxic Shock Syndrome (TSS) except dermal signs. However the cause of such a serious condition was unknown, the possibility of the incomplete type of TSS was suggested. We should promptly cope with the shock state, for example, by administration of appropriate antibiotics and infusion and washing of the local wound. The review of the literature, in-cluding standard case definition, diagnosis and ther-apy was presented.
