頸部捻転により生じた咽頭血腫の1例

A Case of Atraumatic Pharyngeal Hemotoma by Neck Rotation

　はじめに

　咽頭から副咽頭間隙にかけての血腫は，本邦では基礎疾患がある場合や明らかな外傷，または医原性により生じた報告は稀にある。しかし，頸部の捻転後に血腫が生じ呼吸困難をきたした症例報告は，検索し得た範囲では本症を含め2例目である^1）。

　今回われわれは，頸部捻転が原因と考えられる咽頭血腫で，緊急気管切開を施行した症例を経験したので，若干の文献的考察を加え報告する。

An atraumatic pharyngeal hematoma is exces-sively rare.

A 32-year-old healthy man rotated his neck while he was working, he deveroped sore throat, dyspneaand his neck has swollen. Emergency tracheostomy was performed and hematoma was gradually disappeared.

We tried to find the cause of symptoms, and CT, MRI and angiography were performed without success.

Our conclusion was that when he rotated his neck, the ramus of the jugular vein collapsed and hematoma was formed. In case of a pharyngeal hematoma, we should keep an airway immediately.
