輪状甲状靱帯切開後に輪状甲状靱帯開窓術を施行した1例

Cricothyroid fenestration after cricothyroidotomy：a case report

はじめに

　精神遅滞を有する患者を治療する場合，病識や理解力が乏しいため標準的な治療を行えないことが多い。また，セルフケアは十分には行われておらず，肥満を代表とする生活習慣病や齲歯に罹患しやすく重症化する傾向がある。今回，下顎の齲歯に起因するガス産生を伴う頸部蜂窩織炎のため気道狭窄を生じた症例に対し，輪状甲状靱帯開窓術を施行することにより，早期に元通りの生活に戻ることが可能であった症例を経験したので報告する。

　A morbidly obese 24-year-old man with a history of severe mental retardation suffered asphyxia due to cervical cellulitis caused by gas-producing bacteria. He received invasive emergency airway maintenance. When alert, the patient's severe psychological state impeded airway maintenance using tools such as tracheal cannula, and tracheotomy could not be performed because of extremely thick subcutaneous tissue caused by morbid obesity and cellulitis. Out of necessity, the airway was secured by performing cricothyroidotomy and keeping the wound open with absorbable sutures. As a result, the subject could return to his former life without any complications.
