はじめに
未分化大細胞型リンパ腫は,ホジキン病のReed-Sternberg細胞に特異的とされていたKi-1抗原陽性の大細胞型リンパ腫として報告された点で,Ki-1リンパ腫という名称が用いられていた時期もある。
1994年に発表された悪性リンパ腫のREAL分類では,CD 30(Ki-1抗原)陽性で,大型で未分化な形態を取るT細胞型リンパ腫として規定され,anaplastic large cell lymphoma (ALCL)の名称が採用されている1)。
われわれは最近,頸部腫瘤として発症したALCLの2例を経験したので報告する。
We reported two cases of anaplastic large cell lymphoma (ALCL, Ki-1 lymphoma) with the chief complaint of cervical lymphadenopathy. Case 1 was a 16-year-old male and Ga scinti-gram suggestedfemoral bone involvement of the disease. Case 2 was a 60-year-old male with a gigantic abdominal mass and pleural effusion. Both cases were thought to be systemic (nodal) ALCL because of their aggressive course and immunohistological findings (EMA+). They were diagnosed as stage IV disease, but suc-cessfully treated with chemotherapy (5 and 6 courses of CHOP (-G), respectively). Because ALCL is chemosensitive, aggressive chemotherapy should be employed for the treatment of this dis-ease.
