Holmium：YAGレーザーによる内視鏡下鼻内副鼻腔手術

Endoscopic Sinus Surgery with Holmium:YAG Laser

　はじめに

　慢性副鼻腔炎や術後性上顎嚢胞などの鼻副鼻腔疾患に対する外科的治療法として内視鏡下鼻内手術が確立し，さらに様々なレーザーを応用した内視鏡下手術が報告されている^1,2）。CO2，Nd：YAG，KTP／532レーザーなど従来のレーザーによる鼻副鼻腔手術では，骨蒸散能に乏しく深達度が一定でないなど様々な欠点もあった。Holmium：Yttrium Alminium Garnet （Ho：YAG）レーザーは，その特性から組織侵襲が出血の有無にかかわらず一定で，骨との反応性に富んでおり骨蒸散能に優れている。今回，Ho：YAGレーザーを内視鏡下鼻内副鼻腔手術に応用し有用であったので報告する。

With the development of holmium : YAG laser, a device is available for effective sinus surgery, and precise bone ablation, minimal thermal damage to the surrounding tissue, and fiberoptic transmission became possible. We applied the holmium : YAG laser to endoscopic surgery of chronic sinusitis, paranasal sinus mucoceles, and paranasal sinus mycosis. Bone tissue and soft tissue were easily ablated with minimal bleeding, and mucoceles or paranasal sinuses were opened safely and easily in all cases. The sinus cavity mucosa showed normal appearance several months later. No intraoperative complications were occurred.
