中咽頭前壁および側壁早期癌に対するpull through methodの意義と有用性

The Significance and Usefullness of the Pull Through Method for Early Stage of Frontal and Lateral Mesopharyngeal Cancer

　はじめに

　T1, T2の中咽頭癌治療に対しては放射線治療が主体となっているものと思われる^1〜3）。これは中咽頭癌手術が技術的に複雑なことに加えて，術後に嚥下機能，構音機能の障害発生の可能性，また，一方では同部に多い未分化扁平上皮癌に放射線治療が有効で局所コントロールが可能であるなどの理由と相侯って，頸部郭清術のみならず手術治療が行われないことも多い。しかし，中咽頭の部位によるが，T1症例であっても約半数は頸部リンパ節転移が認められていることも事実である^4）。放射線治療で局所コントロールが可能な場合も多いことを否定するわけではないが，理想は嚥下機能，構音機能障害を惹起することなく根治的手術治療ができることである。

　当科ではこのような観点から，中咽頭癌で最も多い側壁癌および前壁癌のstage IIのみならず，症例にもよるがstage I症例に対しても（Tisを除く），病巣の十分な確認からpull throughmethodを用いて局所郭清に頸部郭清術を併用する手術治療を1994年から行っている。早期中咽頭癌そのものが少なく症例が限定されるが，当科で施行した中咽頭早期癌症例に対する手術術式とその有用性，問題点，予後，術後の嚥下機能，構音機能について検討したので報告する。

Mesopharyngeal carcinoma in early stage of stage I, not including Tis, and stage II has been treated with en bloc local resection including elec-tive radical neck dissection by the pull through method. We described operative procedure and find-ings of post-operative speaking and swallowing function in two patients treated and followed-up for 4 years. They were free from disease, and their speaking and swallowing function were satisfac-tory. This surgical procedure has no unpleasant post-operative complications and is effective in improving the 5-year survival of patients with mesopharyngeal cancer in early stage.
