鼻涙管閉塞症に対する内視鏡下鼻内手術の経験

Two Cases of Obstruction of the Nasolacrimal Duct Treated with the Transnasal Endoscopic Approach

　はじめに

　従来，鼻涙管閉塞症に対する治療は眼科医が主体となり，耳鼻咽喉科医がかかわる機会は少なかった。また，治療法としては主に外切開による涙嚢鼻腔吻合術が行われてきた。ところが，近年，光学医療機器の開発・進歩に伴い，硬性内視鏡を用いた耳鼻咽喉科医による鼻内経由の涙嚢鼻腔開窓術が注目を集めている。しかし，それらに関する報告の多くは海外からのもので^1〜10），本邦においては少数例の報告が散見されるのみである^11〜13）。今回われわれは，わずか2例ではあるが鼻涙管閉塞により涙嚢炎をきたした患者に対して，硬性内視鏡を用いた鼻内手術を行う機会を得たので報告する。

Two cases of obstruction of the nasolacrimal duct were treated with transnasal endoscopicapproach. Under local anesthesia mucous mem-brane and bone, which were located in front of the anterior edge of the middle turbinate, were removed using mucosal knife and rongeur, and nasolacrimal duct was exposed. Then, the duct was opened with a mucosal knife easily and a tube was inserted into the duct for preventing closure of the duct. Clinical course of two cases has been good by the time of report.
