血中副甲状腺ホルモン関連ペプチド高値を認め高カルシウム血症をきたした舌癌の1例

Cancer of the Tongue with High Level of Parathyroid Hormone Related Peptide and Humoral Hypercalcemia:Report of a Case

　はじめに

　悪性腫瘍に伴う高カルシウム血症は，腫瘍の産生するhumoral factorの作用により高カルシウム血症を呈する病態で^1），高度な脱水を伴っていることが多く，ときに緊急性を要することがある^2）。頭頸部などの扁平上皮癌に多いとされるが，舌癌に高カルシウム血症を呈した症例の報告は少ない。

A 55-year-old female with tongue carcinoma underwent left semiglossectomy. One year later, ametastasis was found at the right cervical portion and she was readmitted to the hospital because of poor general condition. Her serum calcium showed high level of 15.0 mg/dl with increased level of parathyroid hormone-related peptide (PTHRP). She was diagnosed as humoral hypercalcemia of malignancy (HHM). Although the therapy for hypercalcemia improved her condition, she passed away due to pneumonia and metastasis. This is the first report that an autopsy was performed for HHM with tongue cancer.
