甲状軟骨に浸潤がみられた頸部リンパ節結核症例

Tuberculous Cervical Lymphadenitis Infiltrated into Thyroid Cartilage:Report of a Case

　はじめに

　結核症は，近年の化学療法の普及，進歩により著明に減少したものの平成4年の新規登録患者は48,956人^1）とわが国最大の感染症であることに変わりはない。また，ここ数年の罹患率減少速度の鈍化，在日外国人の結核発症の問題^2）など，結核感染症の新たな局面の展開があり，結核症は決して過去の疾患ではなく，日常診療上常に念頭におく必要がある疾患と考えられる。

　今回われわれは甲状軟骨に浸潤した稀な頸部リンパ節結核の1例を経験したので報告する。

A 59-year-old male presented with a right cer-vical mass (52×42 mm) without tenderness andskin ulcer of the right upper chest. CT scan showed that the cervical mass destroyed the right thyroid catilage and infiltrated into the larynx. The diagno-sis of tuberculosis was made by clinical findings and pathological studies of biopsy specimen from the skin ulcer. Anti-tuberculosis chemotherapy was very effective. Six months later, the cervical mass almost disappeared and the skin ulcer was healed.
