We have experienced two cases of acinic cell carcinoma of the parotid gland, in which serum amylase level varied with treatment, recurrence or metastasis.

The first case was a 45-years-old man. His serumamylase level was high, and this led to detect a tumor in the left parotid gland. It was 542 SU/dl before the operation, and decreased to 92 SU/dl 2 months after the operation.

The second case was a 57-years-old female. Her amylase level increased to 139 SU/dl with a regional lymphoid metastasis. Then after operation of the lung and lymphoid metastases, it decreased to 47 SU/dl.

In conclusion, amylase in the serum was an useful marker to diagnose recurrence or metastasis of the primary acinic cell carcinoma of the parotid gland.
