脳血管異常を伴ったオスラー病の1例

Hereditary Hemorrhagic Telangiectasia with Cerebrovascular Accident:Report of a Case

　はじめに

　オスラー病は，皮膚，粘膜，内臓の多発性末梢血管拡張と同部位からの反復性出血および家族性発生を主徴とする疾患である。遺伝性出血性末梢血管拡張症（hereditary hemorrhagic telangi-ectasia）と命名されたが，一般的にはオスラー病と呼ばれている^1）。耳鼻咽喉科を受診する動機の多くは，反復する頑固な鼻出血である。今回われわれは，めまい発作を主訴とするオスラー病の1例を経験したので，若干の文献的考察を加え報告する。

A 64-year-old female presented with transient ischemic attack. She had intracerebral hemorrhageat the age of 20. Hereditary hemorrhagic telan-giectasia (HHT) was found on the face, lips, nasal mucosa and hands. CT demonstrated an old cerebral infarction. MRA demonstrated neither the internal carotid artery of the left side from neck to head nor the middle cerebral artery of the left side. We sup-pose the abnormal vessels led to central nervous system (CNS) manifestation. HHT caused cere-brovascular accident.
