顔面基底細胞癌の1例

A Case of Basal Cell Carcinoma of the Face

　はじめに

　基底細胞癌（basal cell carcinoma：以下，BCCと略）は，顔面を中心とする頭頸部に好発する悪性腫瘍である。BCCの大きさに関しては，2cm以下のものが約90％であると報告されている^1）。今回われわれは顔面に発生し15年間放置された後，急激に増大し眼窩にまで浸潤したBCCの1例を経験したので，組織学的検討を含め報告する。

A 63-year-old man had suffered from exacerba-tion of left forehead ulcer in the last 1 year after he had it for 15 years. Biopsy was done and pathologi-cal diagnosis was basal cell carcinoma. Tumor invaded bulbar conjunctiva, so he underwent a sur-gical removal of the tumor and left eye-ball. HE stain showed various type of Lever's classification. And immunohistochemical examination using anti-cytokeratin-13 (CK-13) antibody and-19 (CK-19) antibody was performed. CK-19 was localized in the most of tumor cells. CK-13 was identified in small number of cells. Positive staining of CK-13 suggests that the tumor shows the differentiation to the squamous epithelium, and this should be accounted for acute progression of the tumor.
