喉頭癌に対する光線力学的療法後の音声評価

Voice Evaluation after Photodynamic Therapy for Laryngeal Cancer

　はじめに

　声帯に限局するT1，T2の喉頭癌に対する治療は従来より広く放射線治療が中心となって行われてきた。しかし，照射によって腫瘍のみならず正常な粘膜も少なからず障害されるため，放射線治療終了後も声帯の発赤や血管拡張などが認められ，長期間観察していると瘢痕収縮などを起こし声帯振動の低下をきたす症例に遭遇する。その結果，患者は放射線治療終了後も長期にわたり音声障害を訴えることになる。

　当施設では喉頭癌に対し腫瘍のみを選択的に治療できる光線力学的療法（photo-dynamic ther-apy，以下PDT）を行ってきた。そこで，PDT後の音声と放射線療法後の音声を比較検討したので報告する。

The patients with T1 vocal cord cancer were treated with radiation and photodynamic therapies. After the treatment voice disturbance were evaluat-ed by the following parameters : perceptual impres-sion, fundamental period perturbation, peak ampli-tude perturbation and rates of voiced sounds. The results were compared with the measurement results from normal volunteers. As a result of this comparative study, there was a general tendency that those who received photodynamic therapy showed milder voice disturbance than those who received radiation therapy.
