視神経管開放術を施行した外傷性視力障害

Optic Canal Fracture due to Head Injury

　はじめに

　視神経管骨折は外傷性視力障害のうち80％以上に認められるという報告もあり^1），救急外来診療において常に念頭に置いておかなければならない疾患の1つである。視神経管骨折があり，高度の視力障害が認められる場合には，積極的に視神経管開放術を行う必要がある。著者らは外傷による視神経管骨折を強く疑い，鼻内法にて鼻用硬性内視鏡下（以下，鼻内視鏡下と略す）に視神経管開放術を施行した症例を2例経験したので，若干の考察を加えて報告する。

We treated two patients who had serious visual disturbance due to optic canal fracture. Optic nerve was decompressed by the endoscopic endonasal approach. Both patients had no discrimination to light before surgery. One patient unfortunately lost his vision but the other patient restored normal visual acuity at four months after surgery.

CT is thought to be very helpful and essential for diagnosis. Once the optic canal fracture was suspect-ed in CT, optic nerve decompression should be performed as soon as possible after injury.
