A 21-year-old male patient with advanced cer-vicothoracic esophageal cancer presented with the chief complaint of an abnormal sensation in the pharyngolaryngeal region. Roentgenological exami-nation revealed a stenotic region in the cervicothor-acic region of the esophagus (Iu-Ce). The his-tological diagnosis of endoscopically biopsied specimen was poorly differentiated squamous cell carcinoma. To maintain the patient's quality of life, total esophagectomy without laryngectomy was performed after preoperative neo-adjuvant radia-tion and chemotherapy.

Very few cases of esophageal cancer in patients under the age of 29 have been reported in Japan. To our knowledge, the present case may be the young-est of cervicothoracic esophageal cancer in this country. Early esophagography is very important in the examination of even young patients with com-plaints of an abnormal sensation in the pharyngolar-yngeal region.
