心因により耳下腺部腫脹を反復した小児症例

Recurrent Parotid Swelling due to Psychologic Stress:Report of a Case

　はじめに

　耳下腺部の腫脹をきたす疾患は，これまで種々報告されているが，その原因の多くが感染，腫瘍，医原性によるものである。特に，小児においてはムンプスに罹患する頻度が高く，また細菌性感染も生じやすい。反復して耳下腺炎を繰り返す場合は，先天的に耳下腺管の拡張が存在し，逆行性に細菌感染が起こる反復性耳下腺炎を念頭におかなくてはならない。

　一方，学童期は，学校や家庭における種々のストレスから心理的葛藤を生じ，身体にさまざまな心因反応が生じる時期でもある。今回われわれは，心理的要因から耳下腺腫脹を繰り返し，さらに意図的に耳下腺気腫を生じさせていた小児例を経験したので，文献的考察を加え報告する。

A 10-year-old boy complained of recurrent swell-ing of both parotid regions. Blood analysis showed no elevation of white blood cell count, CRP or amylase. Radiologic evaluation disclosed pneumo-parotitis associated with subcutaneous emphysema. Further investigation revealed that the psychologi-cal problems accounted for the recurrent swelling and that he self-inflated his parotid glands. Self-induced pneumoparotitis has been reported to be a condition found in adolescents who often have psychosocial problems. Relieving the patient from tension in addition to quitting self-inflation leaded to the improvement of the recurrent parotid swell-ing.
