耳下腺に発生したepithelial-myoepithelial carcinomaの1例

Epithelial-myoepithelial Carcinoma of the Parotid Gland:A Case Report

　はじめに

　唾液腺に発生する上皮性腫瘍の大部分は多形腺腫である。悪性腫瘍としては，粘表皮癌，腺様嚢胞癌，扁平上皮癌，腺房細胞腫，多形腺腫内癌などが挙げられる。一方，epithelial-myoepithelialcarcinomaは1972年にDonathら^1）が命名した組織学的に上皮と筋上皮の細胞が2相性の増殖を特徴とする低悪性癌である。今回われわれは，耳下腺に発生した本腫瘍の症例を経験したので，臨床的特徴について考察を加えて報告する。

A 68-year-old male complained of a mass in the right parotid gland. Imaging study including sialo-graphy and CT scan suggested malingnant tumor. Total parotidectomy with resection of surrounding lymph nodes was performed and the postoperative histopathological findings showed epithelial-myoepithelial carcinoma. There was no evidence of recurrence nor metastasis for 5 years after the ini-tial surgery. Although this carcinoma is considered as a low grade malignancy, its clinical features suggest that it should be treated as carefully as a highly malignant tumor.
