喉頭狭窄を呈した再発性軟骨炎の1例

Relapsing Polychondritis associated with Laryngeal Stenosis:A Case Report

　緒言

　再発性軟骨炎は系統的に全身の軟骨組織を侵す自己免疫疾患で，1923年Wartenhorst^1）により初めて記載され，現在までの約70年間に世界で約300例が報告されている比較的まれな疾患である^2）。われわれは，感冒罹患後，両側耳介軟骨炎・鼻中隔膿瘍にて発症しその後，喉頭狭窄による呼吸困難を呈した本疾患例を経験した。再発性軟骨炎の病因をめぐる最近のトピックスや自己免疫疾患発症におけるスーパー抗原の関与について，文献的考察を加えて報告する。

A 54-year-old woman was referred for treatment of auricular chondritis and abscess of the nasal septum. Three months later, she developed dyspnea caused by laryngeal chondritis. Multiple chondritis together with histologic confirmation established a diagnosis of relapsing polychondritis. After tra-cheostomy, steroid therapy well controlled chondral inflammation.
