We report the pathological changes of the tempo-ral bone and brain in an AIDS patient who died of cerebral hemorrhage.

In the brain, megaloencephalic hemorrhage and diffuse subarachnoid hemorrhage were found. Multifocal giant cell encephalitis was also detected, which is characteristic of HIV-related ence-phalopathy.

In the bilateral temporal bones, infiltration of erythrocytes was found to extend from the internal auditory meatus of the modiolus into the facial nerve canal. This seemed to be due to spread from the cerebral and subarachnoid hemorrhage.

The extension of cerebral hemorrhage and subar-achnoid hemorrhage into the temporal bone is fre-quently detected in Japanese patients with HIV infection, since there are many hemophiliacs among the HIV-infected individuals in Japan.
