　はじめに

　AIDS （acquired immunodeficiency syn-drome）は，HIV（human immunodeficiency virus）の免疫系や神経系への感染がもととなり発症する疾患である。日本におけるAIDS患者およびHIV感染者の数は確実に増加の一途をたどっており，それに伴い耳鼻咽喉科医がHIV感染者の診療を行う機会も増えてきているといえよう。AIDS患者およびARC （AIDS-related com-plex）患者は種々の耳疾患（中耳炎，感音難聴など）を高率に有するという報告もあり^1），AIDSは耳鼻咽喉科医にとって軽視することのできない疾患である。しかし，AIDSはまだ発見されて間もない疾患であるためか，AIDSの側頭骨病理についての報告は少なく，特に本邦では，これまで報告論文はない。今回われわれは，血友病Aに合併したAIDS患者の側頭骨病理，ならびに脳病理について調べたので報告する。

We report the pathological changes of the tempo-ral bone and brain in an AIDS patient who died of cerebral hemorrhage.

In the brain, megaloencephalic hemorrhage and diffuse subarachnoid hemorrhage were found. Multifocal giant cell encephalitis was also detected, which is characteristic of HIV-related ence-phalopathy.

In the bilateral temporal bones, infiltration of erythrocytes was found to extend from the internal auditory meatus of the modiolus into the facial nerve canal. This seemed to be due to spread from the cerebral and subarachnoid hemorrhage.

The extension of cerebral hemorrhage and subar-achnoid hemorrhage into the temporal bone is fre-quently detected in Japanese patients with HIV infection, since there are many hemophiliacs among the HIV-infected individuals in Japan.
