Ultrasonographic Diagnosis of the Cervical Lymphnode Metastases

　はじめに

　超音波断層法（ultrasonography：以下US）の普及により，リンパ節腫脹とりわけリンパ節転移の診断精度は飛躍的に向上し，客観的な記録が可能となった。USを使いこなすことで，最小限の侵襲により的確な診断をつけられるのは間違いのないところだが，一方で問題点もある。本論では具体的な症例を提示し，USによって得られたリンパ節転移に関する情報を詳細に記述して，現時点におけるUSの有用性と問題点について述べる。

Ultrasonography (US) is a very useful examina-tion to evaluate cervical lymphnode metastases in head and neck cancer. The author reported a case of supraglottic laryngeal carcinoma with bilateral cervical lymphnode metastases (T2N2cM0) and discussed the utility and the problem of US.

US was an essential tool in the accurate staging of lymphnode metastases, because US proved more reliablthan palpation, CT and MRI for evaluating the location and size of lymphnodes.

But the major limitations of US included the differentiation of metastatic from non-metastatic nodes when the size of lymphnode detected by US was small.
