鼓室内顔面神経鞘腫の1症例

A Case of Intratympanic Facial Nerve Neurinoma

　はじめに

　顔面神経鞘腫は日常の診療上，比較的まれな疾患であり，その早期診断は一般に困難とされている。最近われわれは，左耳痛と同側の急激な感音難聴を主訴に受診し，さらに経過中，左顔面神経麻痺を呈し，鼓膜切開下の生検にて鼓室内顔面神経鞘腫を診断し得た1症例を経験した。本症例では10年前にも左顔面神経麻痺に罹患し，保存的治療によって治癒した既往がある。本症例における臨床経過および手術所見を中心に若干の考察を加えて報告する。

A 36-year-old female with a facial nerve neur-inoma in the tympanum is reported. The symptoms of the patient was hearing loss, otalgia and recur-rent facial palsy on the affected side. The pathologi-cal diagnosis was given by the biopsy of the tumor that was observed through the tympanic membrane. Surgical removal of the tumor that located in the horizontal portion was performed. The great auricu-lar nerve graft was transplanted at the defective part of the normal nerve. One year after the opera-tion, recurrence of the tumor was not found.
