顔面頭蓋に広範囲に進展した脂肪肉腫に対する手術的アプローチ

Surgical Approach to Expansive Liposarcoma Involving the Pharyngomaxillary Space

　はじめに

　顔面頭蓋に広範に進展した頬部腫瘍に対する手術的アプローチは，複雑な局所解剖および顔面神経の走行のために困難を伴う場合がある。しかし，近年，頭蓋底外科の進歩に伴い，以前は手術的に摘出不可能とされていた腫瘍に対しても，積極的に手術的アプローチを行うことが多くなってきている。

　今回われわれは，30年以上もの長期間にわたって再発を繰り返し，頬部から側頭下窩に進展した脂肪肉腫^1）に対し，若干の工夫を加えた口腔内切開法および外切開法を組み合わせたアプローチで腫瘍の完全摘出を行い，良好な結果を得たのでその手術手技を絡介する。

A 78-year-old woman who was previously diagnosed as having liposarcoma and recurred 7 times after excisions in the last 38 years was report-ed. The recurrent tumor was complicated with involvement of the pharyngomaxillary spacebeneath the outer skull base from the cheek region.

A combined approach including extended trans-parotid and transpharyngeal approach with midline mandibulotomy was employed, and extirpation of the tumor was completed successfully.

We considered that this surgical approach might provide a safe and complete extirpation of the tumor involving the pharyngomaxillary space with preservation of the facial nerve function and reduc-tion of the cosmetic deformity.
